The following is a summary of covered services provided by DeltaCare. All benefits are subject to the exclusions, limitations and administrative policies of DeltaCare. The standard DeltaCare plan designs and schedule of patient copayments are included in an information kit available through our Network Coordinator. Diagnostic and Preventive Services

• X-rays

• Examinations

• Consultations

• Prophylaxis (cleaning)

• Topical preventive applications (fluoride)

• Space maintainers

Basic Services

• Amalgam (silver filling) and composite (white filling) restorations

• Oral Surgery - procedures for extractions and other surgery

• Endodontics - procedures for pupal therapy and root canal filling (non-vital teeth)

• Periodontics - procedures for treatment of tissues supporting the teeth

Major Services

• Crowns and Fixed Bridges - covered only when a partial denture is not satisfactory

• Removable Prosthetics - full or partial dentures

Orthodontic Services

• 24 months of treatment for comprehensive orthodontics - depending on group contract

Emergency Care

• Emergency Services - immediate relief of pain or treatment of infection

• For out-of-area dental emergencies, Flagship Dental Plans will cover dental expenses incurred up to a maximum amount. "Out-of-area" means that the member is 35 miles or more from their primary care dental office.

DeltaCare coverage is provided by Flagship Dental Plans. This chart provides only certain highlights of plan coverage. As with all health plans, there are exclusions and limitations.
